救助贫困生捐款管理协议书

甲方 : 竹乡行者(以下简称甲方)

乙方 :x乡政府(以下简称乙方)

为弘扬中华民族乐善好施、扶危济困的传统美德 , 由竹乡行者部分网友发起 “ 救助高笋乡中心校一年级贫困生赵xx ” 的倡议以来，共收到驴友、在校学生和社会各界人士捐款共计元，捐物 150 件(套)。根据《中华人民共和国公益事业捐赠法》的规定，为加强对救助资金的管理 , 经甲乙双方协商 , 达成如下协议 :

一、救助金名称

本募捐资金定名为 “ 竹乡行者救助贫困生赵xx捐款 ”，由甲方将募集的人民币元交给乙方代管。本捐款注入乙方后，乙方根据赵xx学习、生活的需要，每年 1 月和 7 月分两次列支救助金 600 元，其中(2011 年列支 200 元)。特殊情况下可提前预支，但不得跨年或突破救助金协议额度。

二、救助金管理

(一)乙方指定专人管理赵xx救助金，建立赵xx救助金工作协调机制，根据需要及时协商，落实到位。

(二)乙方根据救助金管理计划，在监护人(教师或家长)的引领下、凭证或凭据发放。

(三)乙方须为赵xx救助金提供优质服务，确保救助金及时足额兑现到救助对象。

(四)甲方有权对救助金的列支情况进行检查、监督。乙方每年 1 月和 7 月分两次将救助金列支明细帐报竹乡行者(代表)确认，竹乡行者(代表)每年至少一次深入实地调查了解赵xx救助金管理使用情况，发现问题及时纠正。

救助重疾患者捐款管理协议书

甲方 :平度市灰埠阳光爱心社(以下简称甲方)乙方：（受救助监护人）：(以下简称乙方)为弘扬中华民族乐善好施、扶危济困的传统美德 , 根据《中华人民共和国公益事业捐赠法》的规定，为加强对救助资金的管理 , 经甲乙双方协商 , 达成如下协：

一、甲方将募集的专项资金按需注入医疗机构治疗资金缺口，本捐款注入后指定专人管理此救助金，建立乙方救助金工作协调机制，根据需要及时协商，落实到位，因甲方是开放型慈善机构，所有募捐的善款只能最大限度使用于疾病治疗，其它比如护理、善后处理此救助金不予承担。

三、乙方必须根据治疗所需合理使用、不得浪费、挪用、存储。

四、甲方有权对救助金的列支情况进行检查、监督，不定期的安排救助金专业监督人深入实地调查了解乙方救助金管理使用情况，发现问题及时纠正。

五、乙方应严格遵守《中华人民共和国公益事业捐赠法》的规定，在受捐助人有以下情况甲方有权撤销救助： 1.另有救助来源无需甲方救助的。2.因重疾或意外致身故的。

3.情况好转能够自足自理无需救助的。

4.利用救助金它用的，如;经商、炒股、赌博、购置不动产等。

三、乙方无需救助时应该主动申请撤销救助金，不得无故拖延和冒领，否则甲方有权追回，如果被救助人治疗期间放弃治疗或身故，未使用完资金应予以撤回，资金注入同时终止。

四、甲、乙双方以慈善事业为载体，携手共创和谐 , 推动慈善事业发展和社会文明进步。

五、本协议未尽事宜 , 双方协商解决。

六、本协议一式二份 , 双方各执一份。

七、本协议经双方授权代表签字后生效。

爱心捐款

星期五的语文课上，花老师告诉同学们:“我们学校三〔3〕班的一位同学得了白血病，需要巨额的治疗费用，学校号召同学们能够伸出友谊之手，捐出自己的零花钱帮助他战胜病魔，重新回到校园。”

回到家里，我把情况告诉了妈妈，决定捐出自己的零花钱，妈妈非常支持我，说我是个有爱心的好孩子。
